I would wager both + lack of exercise. Even with a disease it shouldn't have reached that point.

When your diet contains a thousand calories per bite, it's not that hard. She's also not the only one. I saw a documentary on NatGeo (I think) several years ago about 3 men across the world with the same problem, and that's basically what caused it.
